How to Obtain a Work Permit

For students looking to obtain a work permit, please complete the permit application, provide one acceptable piece of evidence of age, listed below, and return it to the High School Office. 

Acceptable Documents: 

• Transcript of birth certificate
• Baptismal certificate or transcript
• Passport
• Other documentary evidence
• Affidavit of parent or guardian accompanied by physician's statement of opinion as to the age of the minor
